This evidence-based program offers comprehensive, current information about four chronic diseases: A1C (diabetes), Blood pressure, Cholestrol, and Depression. It includes demonstrations of skills needed by Community Health Workers who provide service to persons with these diseases. Paricipants receive hands-on training during the presentation and a certificate upon completion. Organizations may attend scheduled trainings or request that ABCD Training be scheduled for their staff members or other group.
Case managers, social workers, community service workers, counselors, supervisors, program managers, and all other interested individuals should register and attend this annual event. The conference offers Continuing Education Credits and exhibitor space.
