The Wellness Coalition can help if you: 1) have little or no health insurance, or 2) do not have a family doctor, and 3) need help in managing your chronic disease. Wellness Case Management (WCM) is a service that can help you find a doctor and link you with medications and other services you need to manage a chronic disease.
If you have been diagnosed with asthma, diabetes, heart disease, hypertension, obesity, mental illness or another chronic health condition (or if you use the Emergency Room for everyday health care), your Wellness Advocate can help you set wellness goals and work to reach them. WCM is available for eligible persons in the River Region (Montgomery, Autauga, Elmore, Lowndes and Macon counties). There is no cost to you for WCM services.
